Overview
OpnForm is an open-source builder for forms, surveys and workflows. Fields are dragged into place, with conditional logic, calculations, computed fields, themes and either classic or conversational layouts, then shared by link, embed, Notion, custom domain or QR code. Responses route to Slack, Google Sheets, Zapier, Stripe and webhooks, and it runs on OpnForm's cloud, EU hosting or self-hosted.
